What is GBT Prophylaxe essentially about?
For you as a patient, the new KU64 prophylaxis becomes gentler, more pleasant, and more effective. Traditional professional cleaning involved extensive scraping and polishing to somewhat smoothen rough surfaces. Both the ultrasonic air scaler and hand instruments tend to leave a rough surface and remove significant dental substance. Our new professional dental cleaning no longer focuses on intensive scraping, but rather on the gentlest possible cleaning and disinfection, known as biofilm management. Consequently, AirFlow now uses a new powder: a four-carbon sugar alcohol called erythritol. It is related to xylitol but superior. Superior because, in addition to its mechanical cleaning effect, erythritol has a chemical impact on bacteria that remain, for example in root cementum. It inhibits their metabolism, making them mostly harmless. Using this innovative technique, we eliminate the entire harmful biofilm in your mouth.
This fundamentally changes the procedure of the new prophylaxis treatment:

Effective weapon against harmful bacteria
Throughout life, people of all ages repeatedly complain about toothache or gum bleeding. “Patients often ignore these early warning signs with serious consequences,” explains Dr. Stephan Ziegler, senior dentist and founder of KU64 Dental Practice in Berlin. He warns: “From age 35 onwards, the risk of periodontal disease, an inflammation of the tooth-supporting structures, increases significantly, potentially leading to tooth loss and organ damage.” However, a novel prophylaxis system called Guided Biofilm Therapy (GBT) provides effective protection in combating harmful bacteria.
Weak point gumline
Typically triggered by inadequate hygiene, microorganisms multiply rapidly in the oral cavity. Hidden in difficult-to-reach interdental spaces, on the tongue, or beneath loose-fitting dentures, they wait for food to consume. During this metabolic process, these microorganisms release acids that literally attack dental enamel. The gumline is especially vulnerable to this bacterial layer, known as plaque, responding with inflammation. Patients often remain unaware of this subtle, largely painless process, despite typical symptoms like bleeding gums. “Ideal conditions for inflammatory pathogens to enter the body through weakened tissue, even increasing risks of heart attack and stroke,” explains Dr. Ziegler.
Stopping bacteria before they start
If a bacterial infection is identified, dentists traditionally use methods such as professional tooth cleaning (PZR) with abrasive polishing pastes, ultrasonic instruments, or in more severe cases, curettage (removal of bacteria using sharp tools). “Conventional polishing often misses many problem areas and unnecessarily injures gingival tissue, the gums. Patients frequently find this traditional cleaning unpleasant or even painful, often avoiding future appointments,” notes Dr. Ziegler. With advanced periodontitis and deep gum pockets, the dentist scrapes microorganisms away using fine instruments, resulting in the removal of valuable cementum. This outer tissue between tooth and gums plays a critical protective role. If removed, bacteria enter faster. “Additionally, both these methods, as well as cleaning with conventional ultrasonic devices, leave a rough surface behind—ideal for dangerous pathogens to hide and continue multiplying,” stresses the KU64 expert.
Not all prophylaxis is equal
To stop uncontrolled bacterial growth, experts rely on an advanced version of the well-known powder-jet technology known as Air-Flow. Dr. Ziegler from KU64 Berlin is among Germany’s earliest adopters and explains the new concept: “Initially, the biofilm—the bacterial layer—is made visible using a colored solution. Then, the novel air-water-powder mixture containing erythritol removes the existing pathogens. Compared to its predecessor xylitol, erythritol has enhanced properties. On one hand, it strengthens the mechanical cleaning, individually adjustable with finer or coarser powder depending on the oral situation. On the other hand, it disinfects and suppresses metabolism of remaining bacteria in the cementum. This perfect combination polishes tooth surfaces eight times more effectively, shortening treatment to merely 45 minutes. Even a tomato’s skin remains unharmed by this gentle method. Another advantage: patients now find the temperate 40-degree water jet comfortable.” Regular follow-up appointments with the new GBT maintain dental health and prevent diseases.
Minimizing periodontal risk
Early periodontal detection is particularly essential for implant wearers. To maximize our prophylaxis program’s effectiveness, monitoring bone metabolism also makes sense. Undetected diabetes and vitamin D deficiency negatively influence it—both can now be tested.
